# To Add Clinical Forms

A clinical form is a digitalized form used to collect and record information about a patient's medical condition.

The following are the steps to add Clinical forms.

1. 2. Select **Clinical forms** from the **ATTACHMENTS** list.&#x20;

3. On the **Fill Clinical forms** dialog box, fill in any of the following forms depending on the type of scan.
