To Add Clinical Forms

A clinical form is a digitalized form used to collect and record information about a patient's medical condition.

The following are the steps to add Clinical forms.

1. On the bottom-left corner of the Patient Channel, select .

2. Select Clinical forms from the ATTACHMENTS list.

   
3. On the Fill Clinical forms dialog box, fill in any of the following forms depending on the type of scan.

   • Tuberculosis Diagnosis Forms

   • Stoke Evaluation Forms
