# Confirmatory Lab Test - GeneXpert

The following are the steps to fill out the GeneXpert Confirmatory Lab Test.

1. On the **Confirmatory Lab Test - GeneXpert** dialog box, enter the acquisition time and date from the time and date picker options.

2. Select the GeneXpert lab test results checkbox.
3. Click **Choose file** and navigate to the file location to upload the GeneXpert lab test report.&#x20;

4. If necessary, type additional **Notes** in the empty field.
5. Click **Submit**. The form will appear in a text format on the patient channel.&#x20;

6. Click the text format form to edit.
7. To reset the form, click **Reset**. Click **Reset** on the **Reset this form?** dialog box to confirm the action.
