# TB Vouchers
Few government and healthcare institutions have initiated a TB Voucher program that provides free chest X-rays for patients with TB symptoms. The TB voucher contains information such as voucher ID, date, patient ID, clinic name, doctor's name, etc.
The following are the steps to fill out the form.
1. On the **TB Voucher** dialog box, enter the time and date from the time and date picker options.
2. Type the **Voucher Id** of the patient from the TB voucher.
3. Type the **Referring Center** from the TB voucher.
4. Click **Choose file** and navigate to the file location to upload the TB voucher.
5. If necessary, type the Additional Comments in the empty field.
6. Click **Submit**. The form will appear in a text format on the patient channel.
7. Click the text format form to edit.
8. To reset the form, click **Reset**. Click **Reset** on the **Reset this form?** dialog box to confirm the action.
