# Symptoms

The following are the steps to fill out the form.

1. On the **Symptoms** dialog box, select the checkboxes of the patient's symptoms.

2. If necessary, type the **Others** symptoms in the empty field.&#x20;
3. Click **Submit**. The form will appear in a text format on the patient channel.&#x20;

4. Click the text format form to edit.
5. To reset the form, click **Reset**. Click **Reset** on the **Reset this form?** dialog box to confirm the action.
