Symptoms

The following are the steps to fill out the form.

1. On the Symptoms dialog box, select the checkboxes of the patient's symptoms.

   
2. If necessary, type the Others symptoms in the empty field.

3. Click Submit. The form will appear in a text format on the patient channel.

   
4. Click the text format form to edit.

5. To reset the form, click Reset. Click Reset on the Reset this form? dialog box to confirm the action.
