# Clinical History of Stoke Suspects
Clinical history of stroke suspects allows the users to collect the medical information related to a patient suspected of having a stroke.
The following are the steps to fill out the form.
1. On the **Clinical History of Stroke Suspects** form, select the appropriate time and date in the **Last Known Well Time** and **Arrival Time** pop-up box for the patient.
2. Type the patient's **Blood Pressure** level in **mmHG**, SPO2 value in **percentage**, and **Blood Glucose** level in **mg/DL**.
3. Select the patient's history checkboxes and type the **Additional Comments** in the empty field.
4. Click **Submit**. The form will appear in a text format on the patient channel.
5. Click the text format form to edit.
6. To reset the form, click **Reset**. Click **Reset** on the **Reset this form?** dialog box to confirm the action.
