Clinical history of stroke suspects allows the users to collect the medical information related to a patient suspected of having a stroke.
The following are the steps to fill out the form.
On the Clinical History of Stroke Suspects form, select the appropriate time and date in the Last Known Well Time and Arrival Time pop-up box for the patient.
Type the patient's Blood Pressure level in mmHG, SPO2 value in percentage, and Blood Glucose level in mg/DL.
Select the patient's history checkboxes and type the Additional Comments in the empty field.
Click Submit. The form will appear in a text format on the patient channel.
Click the text format form to edit.
To reset the form, click Reset. Click Reset on the Reset this form? dialog box to confirm the action.
